Please review the chapter cancellation policy prior to submitting a request for cancellation. Please use this form to submit any cancellation requests. Once the form is submitted, a refund will be processed based on the PMI Phoenix chapter refund & cancellation policy. Our chapter representative will contact you, if further details are required before a refund can be processed. Please provide any details/supporting information about your request in "Comments" field. You can request cancellation to multiple events by providing event names and confirmation numbers on multiple lines.
